Send a completed copies of this form to the Leader-on-Loan Coordinator by one of the following modes:
1. Email as an attachment using Microsoft Word.(A separate form without lines is available for this)
2. Email the information required by the form in the body of an email message.
3. Fax the form and send a note by email (or phone the coordinator) to make sure it arrived.
4. Mail four (4) copies of the form to the coordinator.

Please note: if a change of plans is necessary, the Leader-on-Loan or the hostess group should contact the other party and inform the LOL Coordinator.
